STRONG FAMILY HEALTH CENTER is looking for an Elder Services Coordinator. Responsible to: Executive Director. Hipaa Clearance: Full. General Responsibilities: It is the goal of Social Services to reduce the incidences of elder abuse by providing. Elder Services Coordinator will do home assessments, referrals, activities and assist with potential elder abuse situations. Provide recreation opportunities to reduce isolation, and increase community awareness. Assists client advocate in maintaining eligibility in third part resources. Assist elders and families to find resources to keep elders safely in their homes. Typical Duties: Responsible for client home safety assessment and case planning. Assists SFHC clients in establishing and maintaining third-party eligibility (Medi-Cal, Medicare, etc.). Assists in processing of Social Service applications. Makes home visits to clients, as needed. Provides advocacy services to clients. Provides client transportation, as necessary. Performs other duties as assigned to meet changing demands and contractual obligations. Knowledge and skills required: High school graduate, or equivalent, with four (4) years’ experience in social services setting with a focus on Elder Services. Must be able to understand and maintain the complete confidential nature of this position. Must have excellent written and verbal communication skills. Must be able to relate to Native American clients. Ability to work effectively with local social service agencies. Working knowledge of the rules governing social service benefits (i.e. Social Security, Medi-Cal, SSI, Medicare, etc.). Ability to communicate and work with various clientele. Must be a positive role model in representing SFHC and have the ability to work cooperatively with Center staff.
